Program Overview
Hospital Medicine Summit (HMS) is an innovative, multi-topic educational initiative designed specifically for hospital-based healthcare providers (hospitalists, internal medicine physicians, physician assistants, nurse practitioners, pharmacists, and nurses). With significant changes occurring in the healthcare system, there is an increased demand on hospital-based healthcare providers to provide care that impacts patients not only in the acute setting, but as they transition back into the community and are managed in the chronic-care healthcare system.
HMS aims to provide important educational updates from leading experts on conditions that are commonly encountered by healthcare providers practicing in the hospital-setting, including chronic obstructive pulmonary disease, heart failure, and hepatic encephalopathy. Now is a critical time for practitioners to align their practices in anticipation of current and future changes in healthcare policy that will affect payment for services in this setting.

Target Audience
This educational initiative has been designed for hospital-based healthcare providers, including physicians, physician assistants, nurse practitioners, registered nurses, and pharmacists. 
Learning Objectives
  • Define value in healthcare and describe an approach for its assesment in the hospital setting
  • Employ innovative in-hospital opportunities to achieve greater value in healthcare
  • Discuss the central components of an effective value-based payment model
Chronic Obstructive Pulmonary Disease (COPD) Session
  • Utilize long-term treatment strategies to reduce hospital readmissions for COPD exacerbations
  • Review the clinical evidence regarding the efficacy and safety of long-acting maintenance regimens for COPD
  • Select medication delivery devices for patients with COPD based upon individual physical and cognitive characteristics
  • Outline a transitional care plan that promotes patient self-management to reduce the risk for future exacerbations and hospital readmissions 
 Hepatic Encephalopathy (HE) Session
  • Describe an approach to the early diagnosis of patients with HE that is consistent with current guideline recommendations
  • Summarize clinical trial data on the efficacy and safety of options for acute treatment and prophylaxis of HE
  • Implement a transitional care plan to prevent future hospitalizations among patients with HE
Heart Failure (HF) Session
  • Utilize an evidence-based approach to the diagnosis and evaluation of patients with HF that is consistent with current guideline recommendations
  • Summarize current clinical evidence regarding the efficacy and safety of new pharmacologic therapies for the treatment of HFrEF
  • Implement guideline-directed medical therapy for patients with HF
  • Identify transitional care strategies to prevent disease progression and future hospitalizations among patients with HF

Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 5.0 MOC points in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.

Upon receipt of the completed activity evaluation form, you will receive an email from within 3 weeks with a link and directions to submit your credit to the NABP CPE Monitor Service.  
Continuing Nursing Education
The maximum number of hours awarded for this Continuing Nursing Education activity is 5.0 contact hours. Pharmacotherapy contact hours for Advance Practice Registered Nurses to be determined.
