Program Overview
 
Hospital Medicine Summit (HMS) is an innovative, multi-topic educational initiative designed specifically for hospital-based healthcare providers (hospitalists, internal medicine physicians, physician assistants, nurse practitioners, pharmacists, and nurses). With significant changes occurring in the healthcare system, there is an increased demand on hospital-based healthcare providers to provide care that impacts patients not only in the acute setting, but as they transition back into the community and are managed in the chronic-care healthcare system.
 
HMS aims to provide important educational updates from leading experts on conditions that are commonly encountered by healthcare providers practicing in the hospital-setting, including chronic obstructive pulmonary disease, heart failure, and interstitial lung disease. Now is a critical time for practitioners to align their practices in anticipation of current and future changes in healthcare policy that will affect payment for services in this setting.
 
Going Green Initiative
 
In an effort to conserve and reduce the use of paper, participant materials for HMS will only be available electronically. Participants are encouraged to bring their laptops, tablets, or other wifi-enabled mobile devices. Wireless high speed Internet will be provided free of charge.

Target Audience
 
This educational initiative has been designed for hospital-based healthcare providers, including physicians, physician assistants, nurse practitioners, registered nurses, and pharmacists. 
 
Learning Objectives
 
Impacting Transitions of Care and Readmission Session
  • Identify the challenges with transitions of care in healthcare
  • Assess readmission challenges
  • Describe the quality and economic impact of transitions of care and readmissions
  • Review evidence-based strategies and opportunities to improve transitions of care and readmissions
Heart Failure (HF) Session
  • Outline an approach to the diagnosis and evaluation of patients with HF that is consistent with evidence-based guidelines
  • Identify the mechanisms of action of new pharmacologic therapies for the treatment of heart failure with ejection fraction (HFrEF) and describe the clinical evidence regarding their efficacy and safety
  • Implement guideline-directed medical therapy for patients with HF
  • Describe effective strategies for improving transitional care to achieve guideline-directed medical therapy (GDMT) and prevent future hospitalization of patients with HF
Chronic Obstructive Pulmonary Disease (COPD) Session
  • Describe long-term management strategies that may reduce COPD-related hospital readmissions
  • Review important comorbidities of COPD as a part of a management strategy to reduce readmissions
  • Incorporate cognitive and physical assessments during inpatient stays and outpatient visits to determine the appropriate device for patients with COPD
  • Improve communication between inpatient and outpatient healthcare providers who manage patients with COPD to better coordinate care transitions
Interstitial Lung Disease (ILD) Session
  • Outline a multidisciplinary approach for the differential diagnosis of ILD
  • Discuss the latest evidence-based guidelines for idiopathic pulmonary fibrosis and review strategies for individualized management of patient with ILD
  • Describe strategies for ensuring adequate communication between clinician and patient over the course of ILD management
Invasive Fungal Infections Session
  • Identify the limitations to diagnosing the three most common fungal organisms causing fungal infections and recognize when empiric treatment should be initiated in patients
  • Review the emerging resistance patterns of fungal infections and the mechanisms that fungal species use to develop resistance
  • Discuss updated invasive fungal treatment guidelines and how to apply the recommendations in the clinical setting

Pharmacists: To receive credits for participating in the 2017 Hospital Medicine Summit, participants must attend the live program on Saturday, October 14, 2017 in Los Angeles, California and complete the evaluation form provided on-site. Upon receipt of a completed activity evaluation form, transcript information will be sent to the NABP CPE Monitor Service within 4 weeks of completion.
